Kegagalan (failure) dapat dikatakan sebagai sebuah fenomena natural dari produk atau proses apapun. Kemunculan kegagalan terkadang sulit untuk bisa diprediksi, sementara sering sekali dampak yang diakibatkan dari kegagalan bersangkutan relatif signifikan terhadap performansi produk atau proses.

Failure Mode and Effect Analysis (FMEA) merupakan tool yang sangat efektif dalam mengelola kegagalan yang umum digunakan di banyak industri. FMEA akan mampu mengidentifikasi potensi kegagalan yang ada di dalam suatu produk atau proses dan kemudian melakukan pembobotan untuk mendapatkan prioritas terhadap potensi kegagalan yang sangat signifikan yang perlu untuk segera ditangani.
