Deskripsi
Banyak terjadi kegagalan dijumpai dalam kelangsungan proses produksi suatu pabrik. Pada umumnya kegagalan itu diawali dengan kejadian–kejadian awal atau intermediate event, yang disebut sebagai incident, yang dinamakan dengan incident scenario.Konsekuensi dari realisasi kegagalan dalam proses dapat berupa: loss of production,loss of money, equipment damage, releasing toxic chemicals, releasing flammable materials, explosion dan human/environment injury. Diantara konsekuensi itu, releasing toxic chemicals merupakan yang paling dahsyat, diikuti oleh fire ataupun explosion. Dalam rangka untuk meminimasi resiko yang terjadi, diperlukan beberapa langkah/tahapan kegiatan antara lain: identifikasi hazard (condition), incident scenarios dari suatu top accident, risk identification, risk consequences, selanjutnya dilakukan planning and action to reduce the risk.
