Course Description

The course teaches participants the specific steps of the process of investigating an incident using TOP-SET. The teaching is reinforced by active participation in several case studies and other learning tasks.

The case studies include a large-scale simulated investigation which gives practical experience in the investigation process and interviewing. Attendees use the full TOP-SET methodology to investigate the incident. As part of the simulation, participants are provided with hands-on experience of the live interviewing of witnesses using cognitive techniques.

Objective

To teach participants how to lead an incident investigation and take part in the investigation using the TOP-SET methodology.

Who should attend?

The Senior Investigator course is an intensive incident investigation course for senior managers. Based on sound educational practice, it is equivalent in intensity to roughly one week’s work on an advanced college or university- level course.

How is it assessed?

Delegates who have successfully completed and passed the multiple choice question test and practical assessment will be awarded with Certification, issued by Kelvin TOP-SET.

Course Teaching Practice

Participants work through several classroom-based case studies, as well as a live case study that provides experience of conducting a full investigation using the TOP-SETmethodology. The course is a high level class in incident investigation, where active discussion with the tutors is encouraged. All experienced investigators, the tutors have high levels of experience in various fields such as law, fire safety, chemicals, explosives, aviation, maritime, medicine, coaching and education.

Learning Outcomes:

1. The TOP-SET process – participants are given an detailed overview of the TOP-SET incident investigation system.

2. Developing a team – there is discussion about how differences in personality, gender, life and career experiences can benefit an investigation team.

3. Initial data collection – participants are taught how to gather information from the incident scene and to prepare an incident statement to allow planning of the investigation to begin.

4. Investigating – participants will be taught to plan and investigate using the TOP-SET methodology, thinking inclusively

5. Interviewing – the techniques of interviewing for incident investigations are explained.

6. Storyboarding – participants are taught to plot the sequence of events that led up to the incident to help establish the context in which the incident occurred, and to refine the information before analysis occurs.

7. Root cause analysis – participants are taught how to produce a root cause analysis that explains why the incident occurred, using a process of continuous discussion among the team.

8. Recommendations and reporting – the fundamental principles of making recommendations and report writing are discussed.

Fundamental to the TOP-SET incident investigation process are :

1. Developing a Team

The tutors aim to raise awareness of how team-working benefits the investigation. Differences in personality, and differences in life and career experience can greatly enhance the quality of an investigation.

2. Initial Data collection

Participants are taught how to gather information from the incident scene, to consider terms of reference and to prepare an incident statement that allows the planning of an investigation to begin.

3. Investigating

Participants will be taught to use a system of over 600 indicators to plan an investigation. Unlike other systems, which may use a checklist or similar closed system, TOP-SET encourages the team to search creatively for leads. This process produces high quality data for analysis.

4. Storyboarding

Plotting the event that led up to an incident by time helps to establish the context in which the incident occured. It also clarifies the sequence of events before analysis takes places

5. Root Cause Analysis

Root Cause Analysis helps the team to explain why an incident occurred. The team think their way through the incident using logic, analysis and continuous discussion to uncover the root causes.

6. Recommendations & Reporting

Clear communication is vital to making sure that action is taken and that the lessons learned from the incident are communicated to the widest possible audience. Participants are introduced to standardised reporting and the importance of clearly communicating the findings

7. Profiling

Profiling the findings of a number of investigations can build up a picture of where failings persistency occur, so that spesific action can be taken to close the gap
